Description
This position is subject to the Temporary Emergency Standard for Covid-19 vaccination ruling and all employees will have to demonstrate vaccination or have a religious or medical exemption on file.
Job Purpose or Objective(s): Provide diabetes self-management education/training (DSMT) for patients to enable them to learn techniques to bring their disease under control, avoid potential complications and bring about lasting improvements in quality of life.
Primary Tasks:
Provide diabetes self-management education including healthy coping, healthy eating, problem solving, being active, taking medication, monitoring, and reducing risks.
Assess patient at various stages of diabetes, with focus on prevention of complications and management of stable diabetes.
Provide on-going assessment and recommendation to the treatment team, patient, and family.
Document all assessments and interventions; including recommendations and goals in EHR.
Assist patients in monitoring blood glucose for the purpose of medication adjustment.
Perform other duties as may be assigned.
Required Education, Skills and Experience:
MINIMUM
Valid Oklahoma RN license
CPR Certification
Strong problem-solving abilities.
Sound clinical knowledge to identify needs of patients
TARGET
Valid Oklahoma RN license
Certified Diabetes Educator
CPR Certification
Strong problem-solving abilities.
Insulin Pump Certified
Sound clinical knowledge to identify needs of patients
2 years’ experience in diabetes care
