Choice Milestones is hiring for the position of Support Coordinator to work with adults with intellectual and developmental disabilities in NJ. This position is available Part Time with the option to eventually become Full Time if interested or you can stay part time if preferred.
We are seeking a self-motivated and compassionate individual with experience working with adults with intellectual and developmental disabilities. You must have excellent writing and communication skills, flexibility, and be dedicated to helping others. Attention to detail and computer skills are also required.
Responsibilities include making annual home visits, quarterly face-to-face contacts, creating Individual Service Plans (ISPs), attending interdisciplinary team meetings, and making regular phone contacts. You will work with individuals and their families to connect them with community resources and supports to help them meet their goals and aspirations.

Qualifications:
- A Bachelor’s degree with a preference for a degree in Psychology, Social Work, or a related human services field.
- A minimum one year experience with individuals with intellectual and developmental disabilities.
- Previous Support Coordination/Case Management experience and completion of DDD's Support Coordination orientation and training
- A valid driver’s license and your own vehicle.

- Bilingual in Spanish
