POSITION SUMMARY
Join the Community Health Partnership of Illinois team as a Community Health Worker!
Note: This is a grant-funded role.
Under the general supervision of the Outreach Supervisor, the Medical Van Driver, who will serve to transport our Outreach Team to locations in communities for residents and their families to facilitate access to care and resources and improve health outcomes.
In this role, our Driver will work across neighborhoods out in the community to address specific target populations, mostly in collaboration with CHP community partners.
The overall goal of CHP’s Community Outreach department is to enhance community health and wellness around specific priority health needs in the communities and surrounding communities in which we serve.
Essential Job Functions:
We are looking for a committed and reliable CDL Driver to transport our Medical/Dental Outreach Teams
Additional Responsibilities:
1. Excellent Customer Service
2. Punctual
3. Willing to engage with our patients and their families
4. Bilingual- English/Spanish preferred, not required
5. Report potentially unsafe conditions immediately
6. Ability to accurately prepare and complete all required paperwork
7. Work together with co-workers to achieve the overall customer service goals
8. Safety conscious
9. Must possess excellent time management and communication skills
Requirements
Required Qualifications:
• Must have CDL and record in good standing
• Must have a valid Driver’s License, good driving record, and reliable transportation.
• Proactive learner that feels comfortable in different environments.
Required Licenses: Class CDL license required. Valid Driver's License is Required. Work Authorization Requirements: United States Work Authorization is Required. Must be 21 years old to apply. Must have reliable transportation to and from work. Must have a clean driving record and pass a DMV records check. Must pass a pre-employment medical/background.
Position Details
• Job Type/FTE: Full-time 1.0 FTE
• Shift: 40 hrs /week, some weekends - Grant Funded
• Pay Range: open / per hour
• Unit/Department: Medical/Outreach
• Outdoor and Vehicle Conditions: Weather and road conditions, travel is dictated by various circumstances
