The Hospice Admissions Registered Nurse is responsible for receiving and processing referrals to the Hospice program. Explaining the Hospice philosophy and program; obtains necessary medical information; obtains certification of the terminal illness (verbal and written) from the primary physician and coordinates screening/admission visits.
Minimum Requirements:
- Current and valid NY State license to practice as a Registered Nurse
- 1 – 2 years of post-licensure experience in an acute care setting
- Good computer and Electronic Medical Records skills.
- Professional and compassionate communication skills
- BSN and prior experience in home care or hospice preferred
