We are looking for a dedicated and passionate full time, part time and PRN Physical Therapist to join our team. A Physical Therapist with Circle of Care will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals.
Duties/Responsibilities:
- Prepare assessments of pediatric patients’ (birth to age 20) conditions, conduct periodic evaluations, and provide summaries. Develop individualized care plans and problem lists, and document clinical/progress notes on the date of each home visit, incorporating them into the patient’s record.
- Participate in the development and periodic revision of the physician’s plan of treatment, ensuring it aligns with the unique needs of pediatric patients in a home health setting.
- Assist the physician in evaluating pediatric patients by conducting diagnostic and prognostic tests for muscle, nerve, joint, and functional abilities specific to children and adolescents.
- Treat pediatric patients to relieve pain and/or develop or restore function, helping them maintain optimal performance in daily activities within the home environment.
- Record and report any reactions to treatment or changes in the pediatric patient's condition to the physician to ensure comprehensive, responsive care.
- Instruct families on the complete physical therapy program, including the safe and effective use of prosthetic, orthotic, and assistive devices such as canes, wheelchairs, and other mobility aids within the home setting.

Education and Experience:
Required:
- Bachelor’s degree in a PT related field required
- Doctor of Physical Therapy from an American Physical Therapy Association (APTA) approved program.
- Licensure or certification by the state of Texas as a PT
Preferred:
- One year of experience preferred
Physical Requirements:
- Ability to stand, sit, and walk for extended periods of time
- Ability to lift and carry up to 50 pounds
- Ability to bend, stoop, and reach
- Fine motor skills to use tools and equipment
- Visual and auditory acuity to assess patients' conditions and communicate with patients and colleagues
