Duties:
The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress
Responsible for clinical and financial assessments of patients prior to admission
Facilitates and actively participates in interdisciplinary planning of clinical care of patients, and assists with outcome report completion
Provides outcome data to payers and Physicians on as needed basis
Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress
Assists with coordination to most appropriate care level
Initiation and implementation of plan modifications as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress
Communicates discharge information to physician and payer with outcome data, as appropriate
Qualifications:
RN or LPN License is preferred
Case Manager experience is preferred
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
Must be knowledgeable of nursing and medical practices and procedures
