The RN (Registered Nurse) Infusion role is a unique RN position providing specialty infusion therapies in the home setting. The role offers full autonomy, independence, and an impact caring for a diverse population of patients while creating rewarding patient relationships. The RN Infusion provides high-quality, skilled, in-home Infusion therapy and completes visit specific patient monitoring and nursing documentation during in-home visits. The RN in the position will communicate with the patient and/or caregiver directly regarding visit scheduling, reconcile medications and supplies to ensure availability for upcoming infusions and communicate directly with prescribing physician and pharmacy staff. Our ideal future caregiver is someone who: • Is flexible to accommodate varying durations of specialty infusions. • Is flexible varying shift lengths. • Is an expert in IV placement, peripheral line placement and skilled in central line and port access/deaccess and maintenance. • Is professional and comfortable providing clinical education and support to patients, family, and caregivers. • Can work both independently and in a team setting. • Infusion experience is a plus. Professional development would include the opportunity to gain knowledge about numerous specialty infusion medications, to earn specialized nursing certifications and to raise nursing competency levels. Nurses that do infusions in the homes need to be flexible since infusions may last 4, 6, 8 or 12 hours. Also, they travel greater distances than the regular home care RN. Weekends and holidays are a possibility if infusions are needed. They may be required to act as a regular homecare RN in cases where they cannot get to their 40-hour work week. Job Details Job Responsibilities: Provides appropriate training, education and infusion pump connection services to home infusion patients to facilitate timely discharge from acute and long term care settings. Collaborates with pharmacists, nurses, dietitians, physicians and pharmacy staff to resolve infusion and supply problems efficiently and effectively. Documents in a timely, thorough and accurate manner in the electronic health record. Triages pump problems and catheter related problems with patients and caregivers. Facilitates the development of infusion policies and procedures for Cleveland Clinic Home Care Services that reflect current, evidence-based guidelines to ensure patient safety. Creates and maintains relationships with case managers, nurse managers and staff from Cleveland Clinic Health System and contractual clients to support the mission of Cleveland Clinic Home Care Services and patients we serve. Develops working relationships with home health agencies in the communities served by Cleveland Clinic home care services, home infusion pharmacy, to facilitate competent home care nursing coverage for our patients. Develops interactive teaching methods for home infusion p0atients to promote safe, independent care. Continuously evaluates infusion products, supplies and protocols to be used in the care of home infusion patients. Actively participates in national nursing, nutrition or infusion organizations advancing the care of home infusion patients. Leads company performance improvement activities including infection control. Participates in competency enhancing activities tied to company goals and objectives. Participates in weekend, holiday and on-call rotations when assigned. Performs other duties as assigned. Education: Graduate of an accredited school of professional nursing. BSN preferred. Masters of Nursing (MSN) preferred. Certifications: Current licensure as Registered Nurse (RN) in the State of Ohio required. Certified Registered Nurse Infusion (CRNI) or Certified Nutrition Support Clinician (CNSC) preferred. If not certified upon hire, must obtain either Certification within two years of hire. Current Basic Life Support (BLS) certification through the American Heart Association (AHA). Valid Ohio Driver’s License upon hire; per policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$300,000 coverage upon hire; per policy 8-175 thereafter. Competencies (Complexity of Work): Skilled at intravenous and enteral access management, infusion supplies and pumps Excellent customer service skills. Self-motivated and the ability to perform duties with minimal supervision. Organization and detail-oriented, excellent written and verbal skills and current computer skills. Work Experience: Recent nursing experience with strong IV skills required. Home health experience preferred. Physical Requirements: Mobility, grasping, bending, reaching, sitting, standing Personal Protective Equipment: Follows standard precautions using personal protective equipment as required.
