The Client Care Coordinator helps match patients with the healthcare services that are right for them and supports current patients and clients with their ongoing healthcare needs. The Client Care Coordinator demonstrates outstanding customer service skills and use of evolving technology to manage a wide range of patient needs and requests, to include patient intake, connection to appropriate resources, navigating the Cleveland Clinic Health System network and coordinating care.

Responsibilities:
- Handles inbound and outbound communications (phone and email) for new and returning patients.
- Educates new patients about their options, articulate key features and benefits for several Cleveland Clinic programs and identify new ways we can help them.
- Manages patient retention outreach (phone and email).
- Recommends services, schedule appointments, and ensure all materials are provided to the patient for the appointment to ensure a seamless experience.
- Coordinates and maintains data base systems to track the patient journey and adherence to operational goals.
- Collaborates with the team to ensure we deliver a high-quality client experience.
Education:
- Bachelor's degree in business or related field.
Certifications:
- None required.

- Requires working with patients, clinical leaders, cross-functional teams, and third parties on a regular basis to support patient workflow.
- Detail-oriented with strong analytic skills.
- Excellent verbal and written communication skills.
- Excellent organizational, prioritizing and time management skills.

- Ability to work well both independently and as a part of a team.
- CRM experience is valued.
Work Experience:
- Minimum of two years relevant experience.
- High School diploma/GED and six years of healthcare experience may substitute the degree requirement.
- Associate’s Degree and four years of related healthcare experience may substitute the degree requirement.
- Healthcare experience is preferred.
- Epic experience preferred.
Physical Requirements:
- Ability to perform work in a stationary position for extended periods.
- Ability to operate a computer and other office equipment.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment (PPE) as required.
