What are the responsibilities and job description for the LVN/ RN Case Manager (Fresno) position at Clinica Sierra Vista?
The LVN/ RN works closely with the CPHW’s and the providers. The case manager is responsible in reviewing the work that has been completed by the CPHW and is also responsible in reviewing the labs that the provider has ordered and making sure there are no abnormalities in the labs. Also, the case manager coordinates with the provider to make sure all provider orders are completed in EHR
Essential Functions:
- Case manages in electronic health care record.
- Responsible for monitoring and case managing and documenting in patient’s electronic health care record; provides direction and consultation to the staff in the case management of patients.
- Reviews lab orders and lab results.
- Tasks provider with any abnormal lab or ultrasound results, to carry out orders.
- Assists in the coordination of the patient’s care with other health professionals as wells as community agencies.
- Maintains patient files and data bases.
- Reviews the work of the CPHW, to assure accuracy in documentation and billing.
- Makes sure all provider referrals are completed.
- Other duties as required.
Other Responsibilities:
- On a scheduled temporary basis may be required to work at any satellite clinic.
Qualifications:
- AA/AS in a health, social science or related field required and/or graduation from an accredited School or License in Vocational Nursing. Must have a license from the California Board of Nursing preferred.
- Minimum one year relevant work experience in related field and/or minimum two years nursing experience (ambulatory/obstetrical nursing preferred).
- Knowledge of community resources desirable.
- Knowledge of socio-economic environmental and cultural factors affecting patient’s health care.
- Ability to work effectively in an interdisciplinary team approach with both professionals and paraprofessionals.
- Ability to communicate effectively both orally and in writing with persons from diverse social, economic and cultural backgrounds.
- Ability to maintain patient dignity and confidentiality.
- Bilingual and biliterate in Spanish desirable.
- Willingness to work non-regular hours as required.
- Minimum of one year supervisory experience.
- Committed to the belief in health care with dignity for all, and that people have the right to health care information and participation in planning their own health care.
- Full vaccination for NOVEL CORONAVIRUS (COVID-19) is required.
