Requirements:

As an Oncology Nurse Navigator, you will be a crucial point of contact for patients 50 years of age or younger. To excel in this role, you must meet the following requirements:

• Acute Care Experience: Prior experience in an acute care hospital is essential to understand the unique needs of our patients.
• Patient Advocacy: Follow patients throughout the continuum of care, ensuring seamless coordination of services, and at times, performing exams and consults.
• Appointment Coordination: Expedite appointments with multidisciplinary teams to minimize delays.
• Referral Coordination: Assist in coordinating appropriate referrals as needed.
• Community Engagement: Connect patients and their families to available community resources and support groups.
• Compliance: Ensure adherence to navigation standards outlined by the Commission on Cancer and the NAPBC.
• Patient Education: Develop and implement patient education programs and tools, as well as support groups for patients and caregivers.

Education & Experience Requirements:

• Registered Nurse
• BS/BSN required; MS/MSN strongly preferred
• Five years of oncology experience required, OCN certification preferred
• Spanish speaking preferred
