Job Summary: The Admissions Coordinator at Colonial Skilled Nursing Facility is responsible for managing the entire admissions process, ensuring a seamless transition for new residents. This role involves coordinating with prospective residents, their families, and various healthcare professionals to facilitate admissions, maintain accurate records, and ensure compliance with all regulatory requirements.
Key Responsibilities:
Handle all aspects of the admissions process, from initial inquiry to completion of admission documentation.
Conduct pre-admission assessments to evaluate the suitability of potential residents.
Coordinate with clinical and administrative staff to ensure a smooth and efficient admission process.
Maintain detailed records of all inquiries, admissions, and discharges.
Foster and maintain positive relationships with referral sources such as hospitals, physicians, and social workers.
Provide comprehensive information and support to families and prospective residents regarding services, costs, and the admission process.
Ensure compliance with all regulatory requirements and standards related to admissions and resident care.
Participate in marketing and community outreach activities to promote Colonial Skilled Nursing Facility.
Assist in the development and implementation of policies and procedures related to admissions.
Qualifications:
Bachelor’s degree in healthcare administration, nursing, social work, or a related field preferred.
Minimum of 2 years of experience in admissions or a related role within a healthcare setting, preferably in skilled nursing or long-term care.
In-depth understanding of Medicare, Medicaid, and insurance procedures and regulations.
Exceptional interpersonal and communication skills, with the ability to interact empathetically and effectively with residents, families, and healthcare professionals.
Strong organizational skills with keen attention to detail.
Ability to work independently as well as collaboratively within a team.
Proficiency in Microsoft Office Suite and experience with electronic health records (EHR) systems.
