Description
The Admissions Clinician is responsible for all aspects of the initial assessment of members and their family situation for intake into appropriate services, including gathering data for a comprehensive psychosocial assessment and completing the service plan. Intakes performed may be for both adults and children. Rapid Response assessments are conducted on children 0-17 years of age and their guardian. The individual in this position must meet all established guidelines and requirements including the timelines for referrals and written documents.
Essential Duties And Responsibilities (with Or Without Accommodation)
Completes intake/rapid response in an office, home and/or community-based setting as appropriate
Completes all necessary paperwork related to enrolling an individual into the Easterseals Blake Intake and Coordination of Care Program.
Uses knowledge and skill in assessment and diagnosis utilizing ICD-10 criteria, DC 0-5 and DSM-V to provide diagnoses or diagnostic impressions for children and adults
Ability to complete a comprehensive psychosocial assessment and create a service plan/treatment goals based off this assessment that addresses identified needs
Identifies and implements standardized tools and assessments to assist in determining appropriate services and/or interventions
Recognizes, assesses, prevents, and responds to a crisis situation
Refers members to appropriate community resources as necessary.
Complete progress notes in Sigmund within 24 hours of service.
Communicate with biological parents, and any other team member to initiate/coordinate services.
Attends and participates in Child and Family Team and Adult Recovery Team meetings as needed/required.
RR Clinician receives DCS referrals and contacts the children’s placement within 24 hours and complete the Rapid Response assessment within 72-hours of referral.
RR Clinician attends and participates in Team Decision Making Meetings and Preliminary Protective Hearings
RR Clinicians is able the Juvenile Court and DCS systems as it pertains to removal and permanency
May carry a case load based on agency need
Requirements
Education:
Master’s/Bachelor’s Degree in a field related to behavioral health
Master’s/Bachelor’s Degree and at least one year of behavioral health or related work/volunteer experience
Associate’s Degree in a related field and at and at least one year of behavioral health or related work/volunteer experience
Associate’s Degree and at least two years of behavioral health or related work/volunteer experience
High School Diploma or GED with at least three years of behavioral health or related work/volunteer experience
Experience
Behavioral health or related work/volunteer experience
Regulatory
Minimum 21 years of age.
Clean criminal background check and the ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card.
Current valid, unrestricted driver’s license, proof of insurance, and a driving record that meets the standards of the agency.
