To thrive as a Full-Time Registered Nurse (RN) with Comfort Home Care, you'll need a blend of vital skills and knowledge! Your expertise in conducting initial and ongoing assessments will help you identify clients' medical needs while gauging their level of functioning and communication abilities. You'll be the mastermind behind initiating a personalized plan of care, with the agility to revise it as required. Proficiency in performing OASIS assessments at the right moments is essential, as is the commitment to implementing appropriate nursing interventions.
Collaboration is key, as you'll coordinate services and keep physicians and team members informed about any changes in clients' conditions. An empathetic approach toward engaging with patients, families, and community resources will enhance planning for care and discharge. Finally, your knack for preparing clinical and progress notes will ensure that all paperwork is submitted on time, keeping our home care services running smoothly and effectively!
Knowledge and skills required for the position are:
- RN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
- RN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
- RN Initiates and performs appropriate nursing interventions.
- RN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
- RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- RN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
