JOB SUMMARY:
Reporting to the Director of Operations, the Client Care Manager is responsible for ensuring overall achievement of revenue objectives for Toms River NJ. The Client Care Manager will routinely interact with other personnel as well as clients, family members, community providers/referral sources, insurance companies, care managers, and other third party payors.
QUALIFICATIONS:
High School diploma or GED required; bachelor's degree preferred.

ESSENTIAL FUNCTIONS:
Strengthen existing client relations to maintain and grow current business.
Through phone and personal intake of leads, responsible for initially determining type and duration of services necessary to meet client needs. Quote rate based on service provided. Qualify lead for in-home visits. Follow through with in-home visits. Manage follow up with prospects on a consistent basis through phone, email and/or mail.
Coordinate cases with nursing staff as needed.
Establish initial client records, loading detailed information into computer system for schedulers to brief caregivers and forwarding billing information to billing coordinator per company standards.
Read notes made in computer system daily and follow up on any item requiring action.
Complete and maintain updated plans of care for clients, coordinating with nurses as required per company standards.
Maintain statistics of weekly referrals and their sources and submit dashboard weekly.
Visit and/or contact clients to keep abreast of client's condition and environment on periodic basis. Assist in matching caregiver qualification and availability to client needs. Contact clients and caregivers for follow-up on service delivery and determine satisfaction of services. Serve as a liaison between the client, caregiver, scheduler and human resources.
Supervise any staff reporting to the Client Care Manager position.
Update employee files to document personnel actions and to provide information for payroll.
Provide support to on-call staff as manager on duty for territory issues.
Interface with other community resources to assure that client needs are adequately addressed.

Excellent interpersonal skills and ability to persuade others
Proficient computer skills (Office, Excel PowerPoint), including ability to learn and maintain records in company software systems.
Extreme attention to detail is essential
Self manager / Self motivated
Knowledge of local and statewide social service providers a plus.
Ability to work flexible hours required, some evenings and weekends.
Ability to be flexible and prioritize in complex situations.
Strategic planning ability to anticipate changing business conditions and take appropriate action
Decision-making skills and ability to handle pressure.

Pay: $55,000.00 per year
