- Overview* Dignity Health’s Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast’s award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.
- Responsibilities* Community Health Navigators provide culturally and linguistically appropriate perinatal and reproductive health education and services to prevent disease and to promote the physical and mental health of at-risk patients
- Qualifications* CHNs must have lived experience that aligns with and provides a connection between our health system and the community or population being served. This may include, but is not limited to shared race, ethnicity, sexual orientation, gender identity, language, or cultural background.
Education: high school diploma or equivalent preferred
Compassionate mindset towards women and infants
Progress towards certification as a Community Health Worker as defined by DHCS is expected within the first 18 months of employment.
CHNs must complete a minimum of 6 hours of additional job training annually.
- Pay Range* $21.70 - $29.84 /hour
