What are the responsibilities and job description for the Registered Nurse - RN position at Community Based Care?
Overview
The Registered Nurse (RN) assess and identify patients' needs, then implement and monitor the patient's medical plan and treatment. They also ensure that patient care is conducted according to our policies and standards, within the highest standard and consumer outcome.
Responsibilities
- Asses, monitor and treat total health needs of individuals on a continuous basis.
- Make recommendations for administrative or programmatic change which may improve health care.
- Supervise LPN's and medication administration.
- Develop and implement a Habilitation Plan for the individuals.
- Maintain a complete and accurate health record for each individual.
- Periodically assess infection control, safety and training issues in each home.
- Recommend and assist with staff training needs.
- Assist pharmacy, ensure adequate levels of prescribed medications are available for each individual.
- Act as a liaison between the physician, pharmacy and interdisciplinary team.
- Will comply with all Corporate policies and procedures, HIPAA guidelines, DHHS rules, and Medicaid state and federal guidelines .
- Will comply with Corporate Compliance Program policies and procedures.
Additional Responsibilities:
- Be aware and adhere to all regulatory standards.
- To ensure the health and safety of individuals.
- Any other related responsibilities as deemed appropriate by supervisor.
Qualifications
- Accredited and current RN license within the state where services are provided.
- Bachelor's degree preferred.
- ICF/MR or experience with persons with disabilities preferred.
Skills/ Experience:
- Requires experience as RN in direct care services or related field.
