Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral health services for children and adults, and substance use disorder treatment, and specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, quality health care regardless of income or health insurance status. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts.

The Digital Navigator will assist patients in accessing and utilizing technology such as telehealth platforms and MyChart to have increased access to healthcare. This role will assist patients in securing reliable access to broadband internet, reliable access to smartphones and/or computers, reliable access to private spaces to carry out telehealth visits, digital literacy in general and knowledge of the telehealth platform and MyChart.   

Responsibilities 

  • Calling and tracking patients in advance of scheduled telehealth visits to ensure they are trained and confident in their ability to participate, and to confirm availability for the visit 
  • Confirming access to the technology modalities available  
  • Arranging to add interpreters to telehealth visits, if needed 
  • Scheduling telehealth visits and provide associated training 
  • Providing education for patients on-request by providers 
  • Assist patients in determining available resources for broadband internet, cell phone and more to ensure access to technology 
  • Assist patients in setting up MyChart accounts and teaching them how to utilize it for communication with their providers, requesting medication refills and scheduling appointments. 
  • Demonstrates understanding and commitment to the mission of the health center.
  • Demonstrates understanding and commitment to the established CHC Values and Standards
  • Other job-related duties as assigned. 

 Qualification/Skills 

  • Educational Level: Associate degree preferred; two years of experience in health and/or mental health related field 
  • Bi-lingual preferred 
  • Ability to work independently and collaboratively with all care professionals 
  • Experience working in community/healthcare setting 
  • Knowledgeable in Motivational Interviewing 
  • Competent computer skills, especially in Microsoft Office Program (Word, Excel) 
  • Experience with Electronic Health Records, preferably Epic; or on the job training 
  • Excellent customer service skills 
  • Ability to travel between all 5 health center locations
