Clinical Management Services is looking to hire compassionate and caring Registered Nurses to join our team of home health professionals! The Home Care Group is a network of home care agencies, across the Midwest and Florida, which has been providing home health care to under served areas for over 30 years! Clinical Management Services is based in DeMotte, Indiana. We are looking for a committed, experienced, and rooted RN to join our team who has knowledge of home care. The position is preferably Full-Time, but Part-Time hours are available.

We provide in-home nursing and therapy services to patients living in and around; Jasper, Newton, and Lake Counties. Position does require travel to and from patient's homes within the counties that we offer service in. Travel reimbursement is offered.

Requirements:
- Graduate of nursing school and currently holding a RN license.
- 1 year of previous nursing experience, preferred
- Previous home health care experience, preferred
- Ability to communicate effectively and work as a part of an interdisciplinary team
- Strong ability to use critical thinking and deduction when performing job tasks and evaluating patient needs
- Ability to consistently connect with patients one-on-one
- Responsible, compassionate, and caring demeanor
Job Responsibilities:
- Providing services in accordance with the patient’s Plan of Care
- Performing initial evaluations, building Plan of Care, and regularly reevaluating patient’s nursing needs
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical and progress notes for each patient visit via EMR
- Preparing summaries of care conferences on patients in a timely manner
- Informing Physician and Director of Clinical/ Administrator services of changes to patient’s condition and/or needs as appropriate
- Counseling the patient and family/significant others in meeting nursing and related needs
- Supervising and teaching other nursing personnel and CNAs.
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition
- Rotating On-Call schedule
