Community Horizons is assisting with the recruitment process of a private in-home caregiver for a wonderful individual in their home. This is an extremely positive work environment.

You can achieve all of this under Self-Directed Caregiving – Less Stress – No Forced Shifts – Work 1:1

Hours: 8 am-8 pm (12 Hour shifts)

Schedule: Monday, Wednesday, Friday and every other Saturday & Sunday Currently seeking 2 – 3 days a week 12-hour shifts

Wage: $16.50 - $17.00 per hour

Job duties include but are not limited to:

· Personal Care (Bathing, dressing, hygiene, and other Activities of daily living)

· Ceiling Lift – sit-to-stand for transfers in & out of bed.

· Skilled Tasks – Nebulizer Treatments, O2 PRN, R.O.M. & tube feeding.

· Repositioning as needed to protect the integrity of the skin

· Interaction/Companionship revolving around the individual's interests. Favorite T.V. shows, likes to be read to, and enjoys bubble balls!

Who is Community Horizons?

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all the hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
