About The Role
As a Nurse Care Manager at Compass Health Network, you will be an integral part of our person-centered integrated care team. In this role, you will provide direct clinical and medical services to individuals enrolled in the Health Care Home program. The position is full-time and benefits eligible, with working hours from Monday to Friday.

About The Role
As a Nurse Care Manager at Compass Health Network, you will be an integral part of our person-centered integrated care team. In this role, you will provide direct clinical and medical services to individuals enrolled in the Health Care Home program. The position is full-time and benefits eligible, with working hours from Monday to Friday.
Key Responsibilities
- Manage a case load of up to 250 Healthcare Home patients
- Develop yearly care plans for assigned clients
- Assist in creating treatment plan healthcare goals for individuals with chronic diseases
- Monitor and educate clients on medical needs
- Provide consultation and education to staff on health conditions
- Collaborate with medical providers on important health information
- Educate clients on medical diseases, treatment, and medication
- Monitor HIT tools for treatment alerts and hospital admissions
- Analyze monthly reports and communicate findings with providers, clinicians, and patients
- Conduct Medication Reconciliations for discharged HCH enrollees
- Assist in UDS measure documentation/tracking/reporting
- Collaborate within a multi-disciplinary team
- Seek culturally competent services to meet diverse consumer needs
- Report to Regional Primary Care Nursing Director and fulfill clinical supervision requirements
- Perform additional duties as assigned
- High School/GED required
- Associate or Bachelor's degree in nursing preferred
- Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
- Registered Nurse (RN) licensure in MO required
