What are the responsibilities and job description for the Healthcare Home Director position at Compass Health Network?
Compensation: $62.009.60/year
The Healthcare Home Director is part of the person centered integrated care team and manages the day-to-day operations of the health care home level of service for enrolled clients throughout the Compass Health service area. This position supervises the Regional HCH Nursing Directors, the HCH Quality Director, and the HCH Care Coordination Director
Responsibilities Include:
• Champions the HCH practice transformation; Works collaboratively with clinical leadership in all areas to ensure that health home activities are integrated into other clinical programs as appropriate
• Oversees the daily operations of the HCH Team and provides direct supervision to the Regional HCH Nursing Directors, the HCH Quality Manager, and the HCH Care Coordination Manager
• Facilitates monthly HCH Leadership team meetings
• Reviews and approves the monthly reports prior to submission to the State (i.e. attestation, implementation, and other monthly reports)
• Develops working relationships with community hospitals and hospital systems regarding access to, and coordination of HCH services for hospital admissions and discharges
• Works with HCH leadership on HCH program development and compliance
• With the DCO, acts as liaison to the DMH Practice Coaches
• Provide input into the development of agency goals
Supervisory Responsibilities: 10%
• Provide supervision to all direct reports
• Assure adequate staffing patterns to meet needs
• Recruit, select, and orient new staff as needed
• Disseminate information regarding policies and procedures to staff
• Conduct AIM discussions with direct report staff
• Conduct weekly supervision with direct report staff
• Will ensure that staff seek culturally competent services and service providers to meet the unique cultural needs of each agency consumer and family member.
• Will ensure that staff coordinate integrated health care, including psychiatry, community support, primary care & pediatric services, pain management, dental and community based services.
• Attends and assures that HCH staff participate in agency and state required meetings and trainings
Administrative Duties: 5%
• Be a member of the agency’s Management Team and other work groups/committees as assigned
• Attend the State level Health Care Home Director’s meetings and participate on work groups as assigned
Other Responsibilities: 5%
• Attend all training as required by Crider Health Center, Accreditors and Funders.
• Regular attendance except as excused
• All other duties as assigned by the DCO
Job Requirements
Qualifications
• Master’s degree in Mental Health related field (licensure preferred); or Registered Nurse (RN)
• At least five (5) years experience in mental health treatment with those with a serious mental illness and co-occurring disorders
• At least two (2) years supervisory experience
• Knowledge of Adult Community Support program considered a plus
