Hybrid - RN Supervisor of Care Management - Managed Care

Compass Healthcare Consulting & Placement a Healthcare Recruitment Company is conducting a search for an experienced RN Supervisor of Care Management for a Hybrid/Remote position, for a Managed Care Plan MLTC. Must be able to commute to Manhattan office on monthly basis or when needed. Qualified RN Candidates will have prior Registered Nurse RN Clinical Team Lead or Supervisor experience and have at minimum 2-3 years of Managed Long Term Care Plan RN Care Management. Position requires a NYS RN License, prior Managed Care, Care Management experience as well as prior experience Supervising RN Care Managers. This position is mostly remote and requires occasional in-person work within the Manhattan office.

Hybrid - RN Supervisor of Care Management - Registered Nurse

• The Registered Nurse RN Supervisor of Care Management provides clinical guidance and supervision to case management programs, based on accepted principles of nursing, social work, gerontology, geriatrics and case management practice.
• Provides clinical perspective and best practices to staff through one-on-one coaching, group interaction at case conferences, in-service training, and other team meetings.
• Oversees and manages staff responsible for patient care coordination and management which includes case managers, licensed social workers, case management associates and administrators.
• Monitors department activities to evaluate the productivity and quality of programs and processes in order to identify potential improvements and to ensure maximum performance.
• Develops and implements case management and disease management programs that meet the needs of our members.
• Ensures compliance with MLTC & NYS guidelines.
• Develops and maintains professional networks and individual relationships with hospitals, physicians, and other providers in order to promote continuity and quality of care.
• Facilitates communication with providers to ensure continuity of care and coordination between multiple specialists, providers, and vendors.
• Enhances clinical quality of networked medical groups by training medical group staffs upon request.
• Ensures case management team meets established performance metrics and performance guarantees.

Minimum Qualifications

• Minimum of 2-5 years RN Care Management experience.
• Experience working as a Team Leader, Clinical Manager or Supervisor responsible for oversight of Registered Nurse Care Managers required.
• Excellent Leadership Skills Required.
• Ability to track, follow and implement Matrix designed by organization.
• Must have current NYS RN License
• Certified Case Manager (CCM) preferred.
• Minimum of 2-3 years Supervisory experience and leadership of a Case Management staff within a Managed Care organization, Home Care or Hospital environment.
• Knowledge of standard case management concepts, practices and procedures in a Managed Care setting
• Proficient in Microsoft Office; Strong data analytical skills
• Ability to work from home and commute to Manhattan office on occasion.
