Compass, Inc. is a non-profit human services organization that supports individuals with intellectual and developmental disabilities. We are seeking a highly skilled, self-starter with prior experience in human services and/or affordable housing/social services benefits for the position Person-Centered Planning Coordinator. This position’s primary role is to provide support and technical assistance to program staff as well as reviewing Person-Centered Plans (PCPs) against services delivered by Compass. This position works with internal and external stakeholders to keep PCPs on track and to ensure that services are provided and documented as outlined in the PCP. This position reports to the Person-Centered Planning Manager.
Essential Duties and Responsibilities

- Tracks the Person-Centered Planning process to ensure implementation and documentation adheres to regulations and quality standards.
- Monitor the Person-Centered Planning (PCP) process in the agency’s information system iCareManager.
- Utilizes Person-Centered Discovery data to draft meaningful outcomes that accurately reflect individualized goals and dreams of people supported.
- As part of the PCP Planning Phase, completes the Service Implementation Plan and gives it to the Program Coordinator in advance of the PCP meeting.
- Pull data on PCP compliance and work with teams to address any PCPs that are off track or out of compliance.
- Monitor and analyze the data related to Personal Outcome Measures in the CQL Portal and support efforts toward CQL accreditation.
- Utilize the DSAT to complete service authorizations in iCareManager as part of the PCP process.
- Monitor LTSS daily for pending plan activity and acts accordingly.
- Oversees service authorizations and guides teams in maximizing billable services and ensuring service flexibility during the PCP process.
- Compares requested service units as outlined in each person’s plan in iCareManager as submitted to data entered into LTSS and confirms accuracy.
- Troubleshoot PCP issues with teams individually and wholistically, while evaluating and continuously improving company processes globally.
- Engage with CCS and DDA on a routine basis to keep PCP’s moving forward, follow up on any errors and work with teams and leadership to resolve issues.
- Monitor utilization of PCP services through regular audits of service notes to ensure that people are receiving the services they are authorized for.
- Monitor service provision for over and/or underutilization of units and support programs by providing data in these instances.
- Provide training on the PCP process to staff in different roles across the agency.
- Work with LTSS & ISAS to resolve any issues.
- Track staff training compliance with PCP’s in iCareManager, reporting out to programs on staff outstanding training and overall compliance on Person Centered Plan training.
- Prepare required reports, presentations and program documents.
- Participate in organizational and state LTSS-related trainings, webinars and meetings as designated.

Qualifications
Bachelor’s degree in related field. Minimum of 2-4 years of experience in program development and implementation. Experience providing community services to people with intellectual and developmental disabilities preferred. Basic computer skills and strong organizational skills required.
Compensation and Work Conditions
Starting pay is $29 - $31 an hour depending on qualifications. Excellent benefits include health, dental, vision, short term disability and life insurance as well as 401K retirement plan with generous employer match. Paid holidays and 15 days of paid time off (PTO.) The hours of this position are generally M-F 9a-5pm but flexing of hours to enable working some evenings or weekends as needed may be required.
