Concordia Visiting Nurses is seeking a Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN). The Nurse Liaison is responsible to coordinate all offered services for home healthcare, this may include completing part or all of the admission process for patients. The Nurse Liaison (RN or LPN) partners with the sales team in developing and maintaining referral source relationships.
This position will be based out of our Middlesex Office location and will travel as needed throughout our Cabot, Baden, and South service area.

QUALIFICATIONS for the Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN)
- Current Pennsylvania Licensed Registered Nurse or Pennsylvania License Practical Nurse
- A minimum of three (3) years nursing experience required. Supervisory and acute care discharge planning experience recommended
- Maintains current PA Driver’s License and auto insurance.
- Strong clinical assessment skills.
- Home healthcare experience recommended.
- Able to work independently, demonstrating good judgment.
- Must be accurate and detail oriented. Strong interpersonal skills, respectful, and courteous nature.
DUTIES AND RESPONSIBILITIES for the Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN)
- Provides on-site consultative services to area hospitals and facilities regarding adult and geriatric patients referred for homecare services, and referrals for post acute rehab and skilled nursing facility (SNF) care.
- Provides comprehensive adult and geriatric assessments to assure development of thorough homecare plans or discharge plans to Concordia Visiting Nurses.
- Utilizes strong clinical assessments skills to facilitate the development of plans for patients of all ages and medical needs.
- Coordinates/documents adult and geriatric assessments prior to discharge to Concordia Visiting Nurses. Coordinates homecare or post-acute skilled nursing needs to assure quality patient care and delivery of services: helps promote information transfer between levels of care.
- Demonstrates timely and accurate computer documentation/written documentation of patient information as well as daily activities to Concordia Visiting Nurses branch staff via the contact relationship management software (CRM).
- Provides timely response to all intake inquiries at assigned institution; works closely with Intake and Clinical Management.
- Identifies, develops and builds upon referral sources within assigned institution; develops relationships with other referral sources in market area.
- Assists in the identification of new services that are needed or should be expanded based on experience/input at institution.
- Educates markets and promotes to patients, families and referral sources the Concordia Lutheran Ministries continuum of care.
- Provides education, supervision and counseling to the adult and geriatric patient/family regarding expectations of homecare plans and discharge to SNF for post-acute care.
