Job Description
Full-time position with benefits NOT REMOTE WORK, 36,000 Annually with bonuses.
MUST Reside/live in: St. Mary, Terrebone, Lafourche, St Charles, St John Baptist, St. James, Assumption, Washington, Tangipahoa, St Tammany, St Helena, livingston
Conduct in-home assessments, eligibility MDS-HC and completion of Plans of Care, Provide Information on Louisiana Medicaid Long Term Care Programs, Provide community Referrals.
Principal Duties:
- Provide HCBS program Choice Counseling to requestors
- Explain Long Term Care Programs to applicants (Rights and Responsibilities)
- Complete needs-based assessment
- Develop Person-Centered Plan of Care
General Knowlege:
- Department of Health and Hospitals (DHH)
- Office of Aging and Adult Services
- Bureau of Health Services Financing (BHSF)
- Medicaid
- Office of Citizens with Developmental Disabilities (OCDD)
- Medicaid Home and Community Based Waivers
- Understanding of healthcare in areas related to elderly and disabled adults
Education, Training, and Experience Requirements:
- Bachelor's degree in Health or Human Services field, Social Work, or related discipline, or Registered Nurse Degree
- Minimum of 2 years of health related social and/or human services experience
- Minimum of 1 year experience working with older adjults or persons with disabilities
- Valid Louisiana Driver's License and current auto insurance.

- Has no supervisory responsibilities.
