Client Assessment Specialist-Long Term Care

MUST Reside/live in: St. Mary, Terrebone, Lafourche, St Charles, St John Baptist, St. James, Assumption, Washington, Tangipahoa, St Tammany, St Helena, livingston

Conduct in-home assessments, eligibility MDS-HC and completion of Plans of Care, Provide Information on Louisiana Medicaid Long Term Care Programs, Provide community Referrals.

Principal Duties:

• Provide HCBS program Choice Counseling to requestors
• Explain Long Term Care Programs to applicants (Rights and Responsibilities)
• Complete needs-based assessment
• Develop Person-Centered Plan of Care

General Knowlege:

• Department of Health and Hospitals (DHH)
• Office of Aging and Adult Services
• Bureau of Health Services Financing (BHSF)
• Medicaid
• Office of Citizens with Developmental Disabilities (OCDD)
• Medicaid Home and Community Based Waivers
• Understanding of healthcare in areas related to elderly and disabled adults

Education, Training, and Experience Requirements:

• Bachelor's degree in Health or Human Services field, Social Work, or related discipline, or Registered Nurse Degree
• Minimum of 2 years of health related social and/or human services experience
• Minimum of 1 year experience working with older adjults or persons with disabilities
• Valid Louisiana Driver's License and current auto insurance.
