Overview
Talent Pool - Administrative & ClericalThis position is accountable for accurately abstracting all information needed to complete the newborn infant's birth certificate, affidavit of parentage (if parents are unwed), and fetal death certificates meeting all North Carolina state guidelines for completion, before the mother is discharged from the hospital. Serves as liaison between the patient and the county and state Vital Records offices. The schedule for this position is Monday thru Friday 7:00am - 3:30pm.
Responsibilities
- Counsel parents on birth certificate procedures/NC State laws concerning unwed parents and their responsibilities and alternatives for completion of the AOP; Notarize Affidavits of Parentage after their completion; fax to state office and mail to the child support enforcement office; keep Notary certification current.
- Deliver each Mother's Verification of Facts to the mother's room for review and signature; explain the MyChart proxy process and offer parents to order their child's birth certificate using the Ipad for their order.
- Handle all phone calls, questions, and walk-ins regarding the legalities of the birth certificate process.
- Maintain current knowledge on all State laws governing the birth certificate and fetal death process via attending annual NC Vital Records meetings, receiving NC Vital Record email updates, and by communication from your Associate Director or peer birth registrars.
- On a daily basis, run the Delivery report in Epic and interview each mother; Review and abstract clinical information from the patient's record that is needed to complete the child's birth certificate; Complete an Affidavit of Parentage (AOP) with baby's father for Child Born out of Wedlock, if the mother chooses; Utilize the NC Vital Records software to enter information for the birth certificate and state statistical data; Process fetal death certificates through Vital Records website and obtain physician signature within NC State Law timeframe; Release all birth certificates to the Vital Records Raleigh office within NC state law timeframe; Update baby's name in epic to their given name on the Verification of Facts sheet. Scan and index AOP's and Verification of Facts sheets into the legal medical record.
Qualifications
EDUCATION:Required: Associates Degree in a Health related field RequiredA combination of work experience including training and education may be substituted for a degree.
EXPERIENCE:Required: Experience in Microsoft Office (Word, Excel, Outlook)Preferred: Experience in Health Information Management and/or birth registry, knowledge of a foreign language, and experience working with Epic.
