Home Health Scheduling Coordinator
We are looking for an experienced Scheduler to join our team! Our top priority is to ensure our patients receive the best care in the Sacramento region. As a Scheduler, you will serve as a liaison between patients, vendors, intake, and the clinical staff. Our Scheduler plays a major role in our organization, helping to maintain and deepen the relationships we have developed through our marketing and clinical efforts.
Hours:
- Monday-Friday 8:00 a.m. to 5:00 p.m.
- Remote on-call one weekend a month-additional compensation
Responsibilities:
- Works well with field and in-office team members.
- Ensures geographic scheduling to reduce drive time and to enhance field staff satisfaction.
- Schedule clinical staff for admissions.
- Plot frequencies, and other discipline evaluations.
- Ensure authorizations are tracked, scheduled and performed as per POC and payers requirements.
- Manage and complete patient schedules; assign visits timely to ensure completion of patient care, compliance with orders, and allowance for staff to appropriately prioritize visits daily.
- Reschedule visits for call-outs, vacation coverage, etc.
- Communicates appropriately with patients, affiliated vendors, and other departments.
- Promotes an environment of high morale and teamwork.
- Works with supervisor and other team members towards agency goals.
- Attends and prepares reports for meetings and in-services
- Maintains mature problem solving approach under stressful circumstances.
Requirements:
- Must have basic demonstrated technology skills, including operation of a mobile phone.
- Must possess a passion for helping patients.
- Must be able to multi-task and be a forward thinker.
- Prefer 1-2 years of staffing/scheduling experience.
- Prefer 1-2 years of Skilled Home Health experience.
- Proficiency in medical terminology.
