Responsible for performing care management for patients with complex medical and social needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize patient health across the care continuum. Performs duties over the telephone and in person at variable locations, to meet the patient “where they are” such as patient homes, clinics, hospital rooms etc. This is an opportunity to make a difference in the lives of some of the most vulnerable individuals in our community. Role and Responsibilities:
- Conducts assessments to identify individual needs and develops a specific care plan to address objectives and goals as identified during assessment.
- Facilitates patient access to services appropriate to their health needs.
- Assesses behavioral health needs including mental health and substance use disorder challenges and coordinates services as needed.
- Makes home visits to patients place of residence or other facility when needed for patient assessment or education.
- Monitors and evaluates effectiveness of care management plan and modifies as necessary.
- Interfaces frequently with community partners to advocate for patient’s needs.
- Works closely with a community health worker to address social determinants of health including; transportation, housing, and food security.
- Manages and delegates daily to assigned staff
- Requires ability to facilitate community wide meetings, encouraging collaboration among multiple agencies
- Must reach out to community resources to establish and build working relationships
- Must communicate by telephone frequently (approximately 20 calls a day)
- Must contribute to bi-weekly virtual staff meetings
- Frequent collaboration and meetings
- Occasionally transport patients to appointments, etc.
Qualifications:
Required Qualifications and Experience:
- Current RN license in WA state
- A demonstrated comfort level working with people & situations that can feel frustrating, unclear, & seemingly hopeless; Ability to work with a positive attitude in stressful situations.
- Basic computer skills
- Electronic Medical Record experience
- Valid WA driver’s license & car insurance
- Excellent oral and written communication skills
- Motivational Skills
Preferred Qualifications and Experience
- Medical and behavioral assessment experience
- Motivational interviewing experience
- BSN
- Certified Case Manager (CCM)
- Dual diagnosis experience with mental health and substance abuse
- Home health experience or case management experience
Physical Requirements
- Must be able to remain in a stationary position 50% of the time.
- The person in this position needs to frequently move inside the office to access file cabinets, office machinery, etc.
- The person in this position needs to frequently move outside the office to access vehicles, client houses, hospitals, etc.
- Constantly operates a computer and other office productivity machinery, such as a copy machine, printer, etc.
- The person in this position frequently communicates in person, over the phone, and electronically with clients, co-workers, community partners, etc. Must be able to exchange accurate information in these situations.
- Must be able to observe and assess clients and their living situations.
- Occasionally lifts up to 20 pounds
- Constantly exposed to outdoor environmental conditions during travel between multiple locations
