Position Description:
GENERAL SUMMARY
As a Medical Social Worker in the Social Services Department, you will play a pivotal role in providing support and
assistance to individuals 18 years of age and older facing challenges accessing and identifying resources. You will work
closely with healthcare teams, patients, community agencies, and other stakeholders to develop and implement plans aimed
at achieving optimal outcomes. Medical Social Worker role will encompass a wide range of responsibilities aimed at
addressing the social and emotional factors that impact patients’ well-being and treatment outcome. The Medical Social
Worker will provide assessment, advocacy, crisis and mental health support in a clinical and non-clinical setting. The
Medical Social Worker will possess problem-solving skills in assisting patients navigating the health care system. In
addition to assisting with prescription assistance program, Medical Social Workers will also undertake various other duties
as outlined to support the well-being of our patients within the healthcare setting.
KEY DUTIES & RESPONSIBILITIES
1. Community Engagement: Engage with communities to identify social determinants of health impacting
individuals’ access to care and treatment outcomes.
2. Community Organizing: Collaborate with community organizations, advocacy groups, and local resources to
develop initiatives and programs aimed at addressing social issues affecting health outcomes.
3. Assessment and Intervention: Conduct comprehensive assessments to identify psychosocial needs, case
management and provide interventions tailored to the unique circumstances of each patient and family.
4. Case Management: Coordinating and Facilitating services to meet the complex needs of individuals and families.
5. Advocacy: Advocate for patients’ rights and assist them in navigating the healthcare system, including facilitating
communication with healthcare providers, and addressing barriers to care.
6. Education: Educate patients, families, and caregivers about available resources.
7. Professional Development: Stay abreast of current trends, best practices, and evidence-based interventions in
medical social work through ongoing training, professional development opportunities, and participation in
relevant conferences and workshops.
8. Documentation: Maintain accurate and up-to-date documentation of client assessments, client encounters and
service coordination activities in accordance with department workflow and processes.
9. Supportive interventions: Offer supportive interventions, conflict resolution and coping strategies to help patients
and families navigate the challenges of their medical journey.
Social Services
10. Educate patients and caregivers about our prescription assistance program, eligibility criteria, application
processes, and resources for obtaining medications at reduced cost or free of charge.
11. Resource Navigation: Assist patients in accessing community resources, financial assistance, housing support,
transportation services, and other resources to meet their needs.
