Corporate Guardians is hiring a solution-driven social worker or human services professional with experience with adults with disabilities or the aging population to be a Guardian of Person Representative.
A Corporate Guardians Guardian of Person representative will
- Manage a case load of individuals under guardianship with responsibilities including but not limited to assisting individuals with decisions regarding community support, employment, medical procedures and recommendations and residential placement options.
- Conduct quarterly site visits, attend care plan meetings, member centered plans, employment plan meetings, educational plan meetings, etc.
- Promote team cohesiveness in a solution driven manner for the best interest of the ward.
- Have full knowledge of individuals on case load including support teams, behavioral issues, values and preferences, medical diagnosis, medications, family dynamics, and any other relevant information enabling a well-rounded approach to service.
- Will be trained to have a working knowledge of Wisconsin State Statutes governing guardianship services including Ch. 51, 55, 54 and Administrative Code 85.
- Complete required continuing education as approved by the director and required by the State and share training information at monthly all staff meetings.
- Participate in community involvement committee or activity
- Attend court for guardianship and protective placement hearings.
- Participate in on-call rotation. On-call rotation is shared by all Corporate Guardians' guardian representatives. Each guardian rep is required to cover one week at a time, 24 hours/day 7 days/week. On average, each guardian rep will have 2-3 on-call shifts per year.
Expectations:
- Demonstrate ability to work independently with minimum amount of oversight and direction.
- Represent Corporate Guardians in a professional and courteous manner to clients, team members and the community.
- Advocate for wards' best interests in a positive, solution driven manner and secure additional advocates as deemed appropriate by supervisor.
- Strong written and verbal communication.
- Proficiency in Office 365 environment
- Must pass Wisconsin Caregiver background check, including criminal records check for Wisconsin and previous states of residence.
- Possess awareness of community resources and funding options.
- Must be supportive of fellow Corporate Guardians team members and assist in the coverage of other case loads/duties when needed.
Requirements:
- Possess a four-year bachelor's degree in a human services related field and/or the combined equivalent of training, education and experience as deemed appropriate by director.
- Possess and maintain a valid driver's license

Pay: $17.00 - $23.00 per hour
