ABOUT THE POSITION
Under the direction of the Homelessness and Coordinated Care’s (HCC) Division Director, The HCC Program Manager for Homelessness & Coordinated Care (HCC) ensures on-going successful implementation of the county’s homelessness-focused programs through program, fiscal, and personnel management and builds a sustainable model that adjusts to upcoming changes in the fiscal and policy landscape. The HCC Program Manager is responsible for management of the operations and outcomes of the Division’s homelessness system of care. The HCC Program Manager makes programmatic, budgetary and operational decisions within the programs' strategic and regulatory framework; provides direct management of staff and through subordinate supervisors; and ensures the county has a unified care management system with standardized screening, assessments, and care coordination, while promoting bi-directional information sharing and care coordination among providers.
The Homelessness Program Manager takes an active role in performance management, coaching and staff/team development.
Fixed-Term:
This is a 24 month fixed-term position. This position is budgeted for June 30th, 2026. However, there may be opportunities for the hired individual during this time-period to move to a regular, ongoing position as other vacancies or promotional opportunities occur, or the position may be funded and convert to an ongoing position. Fixed-term employment ends when the position’s funding finishes, or the work is completed. Employees in these positions are exempt from the County’s policies regarding lay-off order and reduction-in-force rights.

The ideal candidate has a broad range of professional level operational expertise that includes program operations, CalAIM/Medicaid, homelessness, safety-net programs, and public policy. As Program Manager, the successful individual must be able to establish effective collaborative relationships and provide leadership and oversight to a strong, seasoned team. The ideal candidate is a strong manager, analytical thinker, public speaker, and an excellent writer. The ideal candidate possesses the political astuteness necessary for navigating a wide variety of internal and external stakeholders while maintaining sensitivity to their competing needs. The ideal candidate has experience developing and managing budgets, especially budgets that include diverse grant funds. Lastly, the future policy and funding landscape for this program are incredibly dynamic. Therefore, the ideal candidate is accustomed to managing multiple funding streams and adjusting program and budget plans to meet changing policy and funding contexts.
QUALIFICATIONS
Knowledge of:
- Principles and practices of management, including program development, implementation and evaluation; goal setting, personnel management; budgeting and fiscal administration; grants administration; and contract negotiation and monitoring.
- Applicable laws, rules, codes and regulations.
- Principles, methods, and practices of employment and training, clinic administration, and community needs and resources.
- Manage, implement and evaluate services that carry out overall program objectives and achieve appropriate outcomes.
- Interpret, explain and apply legislation, regulations, policies and directives; develop and implement appropriate clinic/program policy; and ensure clinic/program compliance.
- Plan, organize and manage the activities of employees.
- Develop and maintain effective working relationships with those contacted during the performance of duties, including multi-disciplinary staff, community members and organizations, public and private agencies, and local, state and federal agencies.
- Work collaboratively to enhance service integration within the department and non-profit community partners.
- Prepare and present effective written and verbal reports, policies, procedures and other written materials.
Minimum Qualifications:
Any combination of education and experience which would provide the required knowledge and skills listed. Typically a Bachelor's degree in public health or health care administration, or a related field and three years of experience appropriate to the assignment including one year of supervisory experience.
