POSITION: SERVICE COORDINATOR I- Early Intervention

Full-Time, 80 hours per pay

$16.12/hr.

OBJECTIVE OF JOB

To provide case management, as well as link to and coordinate Early Intervention and Community services and supports for infants and toddlers, birth to three years old, who demonstrate state and federally identified levels of developmental delay or who are likely, due to diagnosis or circumstances, to develop such delays. To accept and work with any infants, toddlers, and their families who are residents of Venango County and are referred to the Early Intervention (EI) program, regardless of referral source. To provide, either directly or through linkage to, services and supports designed to improve the child’s development.

Receives instruction and supervision from the EI Service Coordinator (SC) Supervisor in regard to work duties.

ESSENTIAL FUNCTIONS OF THE JOB

1. Conducts intakes/initial family interviews/assessments to determine the strengths and needs of infants, toddlers and their families.

2. Provides an initial Ages & Stages Questionnaire (ASQ) screening on all children referred to Early Intervention, if deemed appropriate and agreed upon by the family.

3. Refers at-risk children to the EI Tracking Program, which tracks the development of at-risk children utilizing the ASQ screening tool, if deemed appropriate and agreed upon by the family.

4. Coordinates the performance of initial and ongoing evaluations and assessments in a timely manner.

5. Facilitates and participates in the development, implementation, review and modification of Individualized Family Services Plans (IFSP) on a timely basis

6. Assists the family in identifying available EI service providers. Coordinates the provision of, and communication with, Early Intervention providers as well as other service providers/agency staff that the infant or toddler is referred to and/or receiving services/supports from.

7. Assists the family of an infant or toddler with a delay, disability, and/or otherwise identified as being eligible for Early Intervention in gaining access to Early Intervention services, as well as other services and supports identified on the IFSP.

8. Facilitates the timely delivery of Early Intervention services.

9. Monitors progress on IFSP goals and strategies and makes revisions as needed.

10. Assists in monitoring service delivery and quality of EI services and supports through session note and chart reviews, as well as service delivery observations.

11. Assists the family in arranging for the infant or toddler with a delay/disability to receive medical and health services.

12. Informs the family of appropriate community resources and the availability of advocacy services. Makes referrals to other community services/agencies as necessary.

13. Maintains regular contact with the identified child’s parents/guardians and/or caregivers through home visits, phone contacts, virtual platforms, or written correspondence as required through EI regulations and the specific child and family needs.

14. Ensures that parents of all children eligible for the Infant Toddler Family (ITF) Waiver are properly informed of the availability of that funding source. Completes the proper applications and documentation expeditiously. Prepares and submits waiver paperwork and reports for EI SC Supervisor and Program Manager to review. Collaborates with the local County Assistance Office to ensure that all ITF waiver eligibility documentation is maintained. Ensures that all waiver documentation is entered and updated in the state-wide data base in a timely manner.

15. Facilitates timely transition planning for children exiting the Infant/Toddler EI program, including facilitating the development of a transition plan and meeting as required.

16. Maintains accurate, timely documentation and records on individuals and their progress within their physical chart and on the state-wide data base as required.

17. Maintains Excel caseload spreadsheets and prepares reports monthly and as requested by EI SC Supervisor and Program Manager.

18. Assists with all EI cases, regardless of assigned Service Coordinator, as deemed necessary or requested by EI SC Supervisor.

19. Assists in identifying training needs and opportunities for the EI program, EI families and early childhood community, as well as linking EI Families to training resources.

20. Participates in and documents all relevant or required EI trainings. Maintains timely and accurate documentation of trainings and qualifications in the state-wide Provider Registry data base.

21. Assists with disseminating informational materials, planning, coordinating, and participating in Child Find activities as assigned.

OTHER DUTIES OF THE JOB

1. Attends staffing, training and other meetings as required.

2. Attends certification trainings as required under the PA Department of Human Services Annex A Title 55, Part VI. Mental Health Manual, Chapter 4226 Early Intervention Services, ss 4226.29 Preservice Training and 4226.30 annual training.

3. Performs other job-related work as required.

QUALIFICATIONS

EDUCATION/EXPERIENCE

Associate’s degree majoring in social welfare and 3 years of professional casework or human service experience; or a bachelor’s degree majoring in social welfare and 1 year of professional casework or human service experience; or a bachelor’s degree in an unrelated field with at least 12 credits in social welfare and 2 years of professional casework or human service experience; or a bachelor’s degree and successful completion of an accredited casework intern program.

Clearances:

  • Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.
  • Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
  • Must successfully complete pre-employment drug screening.

7. Must possess the technical knowledge of operating personal computers and other office equipment.

8. Must possess a valid Pennsylvania Driver’s License and daily access to reliable transportation. Travel will be throughout the county to complete assessments, monitor conditions, and provide interventions in individuals’ homes within the county.

9. Must understand and comply with all mandatory reporting requirements of Early Intervention.

10. Must possess the principles and practices used in social service work, and the ability to apply those principles and practices within the scope of the position.

11. Must possess knowledge of social, economic and health problems and resources. Must be able to interact effectively with said resources on the individual’s behalf.

12. Must possess the ability to conduct thorough, complete assessments to determine the most appropriate level of care needed.

13. Must possess the ability to plan and organize work and prepare adequate records and reports.

14. Must possess the ability to practice organizational and stress management skills and to practice use of good judgment.

15. Must possess the ability to deliver and evaluate appropriate services to individuals and to monitor the effectiveness of such services.

16. Must possess some technical knowledge of operating personal computers and related software, or the ability to learn and operate with reasonable accuracy and speed.

17. Must possess ability to interpret regulations, policies and procedures, and apply them accordingly.

18. Must possess the ability to work with persons who have physical, mental, or emotional disabilities, who are economically disadvantaged, involved in the court system, and advocate on their behalf.

19. Must possess knowledge of child development or the ability to obtain and apply such knowledge to assess a child’s strengths and needs.

20. Must possess the ability to research medical conditions and diagnoses in order to assess the impact on child development and share information and resources with families.

21. Must possess the ability to learn to write goals and strategies related to the specific IFSP treatment goals and strategies identified for a child and family.

22. Must be able to interact effectively with internal agency staff, state department staff, therapists, provider agencies, community partners, attorneys, and court.

23. Must possess a high level of collaboration and coordination skills in order to assist in meeting the needs of the child and family.
