CovenantCare at Home is seeking qualified candidates for an Intake Coordinator (RN or LPN) to help in fulfilling our common purpose: to create joy and peace of mind for our clients and their families by creating a better way of life.   • Answer the telephone, field calls and accepting referrals and telephone orders.
  • coordinates all intake calls and screens for appropriateness.
  • entering patient referral information into database when appropriate.
  • responsible for maintaining supply inventory as well as daily distribution of supplies to field staff. Maintains log and any reports essential for appropriate billing reorder process.
  • verify benefits for new and/or existing patients. Obtain authorizations as required.
  • identify staff members with documentation deficiencies and request corrections.
  • actively participates on Quality Improvement Program committee.
  • serve as a resource to DON and field clinical staff.
Job Qualifications:
  • Active RN or LPN license in state applying for.
  • One year of home health experience.
  • Intake coordinator experience preferred.
  • Proficient in Microsoft word, excel, and power point.
  • Strong interpersonal and communication skills
  • Experience with Netsmart/Allscripts EMR preferred.
  • In compliance with state and federal vaccination mandates.
