- Answer the telephone, field calls and accepting referrals and telephone orders.
- coordinates all intake calls and screens for appropriateness.
- entering patient referral information into database when appropriate.
- responsible for maintaining supply inventory as well as daily distribution of supplies to field staff. Maintains log and any reports essential for appropriate billing reorder process.
- verify benefits for new and/or existing patients. Obtain authorizations as required.
- identify staff members with documentation deficiencies and request corrections.
- actively participates on Quality Improvement Program committee.
- serve as a resource to DON and field clinical staff.
- Active RN or LPN license in state applying for.
- One year of home health experience.
- Intake coordinator experience preferred.
- Proficient in Microsoft word, excel, and power point.
- Strong interpersonal and communication skills
- Experience with Netsmart/Allscripts EMR preferred.
- In compliance with state and federal vaccination mandates.
