What are the responsibilities and job description for the Registered Nurse (RN) position at Creative Community Living Services?
Creative Community Living Services is a leading agency providing in home support for adults with disabilities. As our regional nurse through out The Greater Milwaukee area, you will ensure resident's healthcare needs are met.
What You'll Do:
- Conduct initial and ongoing comprehensive assessment of the client’s needs
- Under the guidance of the Chief Nursing Officer, coordinate total patient care by monitoring the client’s condition, promoting sound preventive practice & coordinating services
- Develop, review and update client care plans
- Answer care plan questions, or emerging client concerns and provide guidance as appropriate.
- Follow up on client changes, concerns, and incidents in a timely manner.
- Communicate with family members as required
- Follow safety and quality measures by assessing home environment for adequate supplies/equipment (gait belt, shower chair, etc.) considering any client deconditioning and/or changes in abilities
- Conduct and participate in care conferences.
- Assist with supervision and training of caregivers regarding proper care techniques (observed during visitations)
- Other duties as assigned
Job Type: Full-time
Pay: $30.00 - $33.00 per hour
