Family Foundations Practitioner - Associate Professional
Job Summary: Be a vital part of our mission to empower children and families by becoming a Family Foundations Practitioner. In this role, you will act as the service definition’s Associate Professional role for Crossnore’s Intensive In-Home Team, Family Foundations by providing direct behavioral interventions and case management support to families served by our team in the Hendersonville Region. Travel to community agencies and families’ homes will be required. Position will require after-hours on-call support to children and families, and team members must live locally to the Hendersonville area. Hiring Range: $42,000 to $47,000 annually

Education/Experience
- At minimum, meets requirements for an Associate Professional which are one of the following:
- Graduate of a college or university with a masters degree in a human service field with less than one year of full-time, post-graduate degree accumulated clinical experience with children and families
- Graduate of a college or university with a bachelor's degree in a human service field with less than two years of full-time, post-bachelor's degree accumulated clinical experience with the children and families
- Graduate of a college or university with a bachelor's degree in a field other than human services with less than four years of full-time, post-bachelor's degree accumulated clinical experience with children and families
Skills/Competencies
- Service Delivery Coordination:
- Ensure service delivery aligns with state Intensive In-Home Service Definition.
- Coordinate with team members to meet service definition requirements.
- Maintain appropriate team-to-family ratio.
- Client Support and Intervention:
- Assist in implementing home-based behavioral support plans.
- Provide psychoeducation to clients and families.
- Monitor and evaluate the effectiveness of interventions.
- Crisis Management and Safety:
- Provide coverage of emergency/crisis line.
- Consult with team members regarding safety issues and crisis response.
- Identify risks, promote safety, and implement safety plans.
- Referral and Collaboration:
- Consult with team members and make referrals for needed services.
- Collaborate with various agencies and providers for client support.
- Training and Development:
- Attend and demonstrate competence in required training.
- Participate in ongoing continuing education annually.
- Administrative and Organizational Skills:
- Maintain a flexible work schedule.
- Ensure timely completion of clinical documentation.
- Follow agency protocols and maintain positive relationships with local agencies.
- Internal Operations:
- Participate in ongoing professional development and supervision sessions.
- Ensure personnel file compliance.
- Contribute to a healthy team environment through effective communication and conflict management.
- Compliance and Quality Improvement:
- Work with leadership and PQI Team to meet contractual obligations and quality improvement plans.
- External Operations:
- Collaborate with other programs and services to fulfill organizational mission.
- Develop and maintain relationships with community organizations and referral agencies.
- Independence and Accountability:
- Demonstrate the ability to work independently with sound judgment.

Pre-Hire Requirements:
