Position Qualifications:
- 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care, or Acute Care environments
- LSW required
- Bachelor's Degree Required
- Licensed Master's Level Social Worker Preferred
- License in Ohio Required
- Medicare guideline experience preferred
Job Responsibilities:
- Float will cover caseloads/visits to support patients and families throughout the territory.
- Assess and document patient/family social, emotional, financial, and psychological needs as well as resources on the Comprehensive Assessment
- Assist patient and/or family with establishing DNR, advance directives, and/or DPOA or Healthcare proxy as requested
- Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, and GIP as dictated by patient/family needs
- Work in both Hospice and Palliative care Programs
- Office is located in the Akron/Canton area and travel to the office is required.
Shift:
- Monday to Friday 8:30 AM to 5:00 PM with an on-call rotation that includes evening and weekend hours
