Position Summary: The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring clients' health through the nursing process; collaborating with interdisciplinary team members; providing physical and psychological support to clients, friends, and families.
Scope of Practice: Assessment of the physical needs of the client and implementation of a plan of care through preventative, and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.

Per licensing and certification requirements.
Essential Duties and Responsibilities:
Direct Care:
- Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs.
- Conducts ongoing assessments as determined by patient’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
- Develops plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
- Performs appropriate treatments as ordered by provider in an accurate and timely manner.
- Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family.
- Delivers care with appropriate age and cultural competence to specific patient populations according to individualized needs.
- Provides individualized patient/family education customized to the patient and his/her family.
- Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
- Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
- Maintains confidentiality in matters related to patient, family and client facility staff.
- Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity and rights.
- Reports patient condition to appropriate personnel during each shift.
- Maintains current competency in nursing specialty by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
- Assesses for pain, utilizes pain scale; documents accordingly.
- Completes pertinent status reports by end of shift (i.e. incident, suicide, AWOL, infection control and acuity).
- Completes work left from previous shifts and reports incomplete assignments to ensure continuity of care; finishes patient care responsibilities.
- Establishes and maintains a therapeutic alliance with patients by assisting the patient to use the therapeutic milieu.
- Is proficient in special procedures including, but not limited to collecting laboratory specimens, glucometer, O2 Administration, dressing changes, urinary catheters, and insulin dosage calculation.
- Ensures that lab results and other test results are posted upon receipt. Ensures that physician orders are affected within 30 minutes and transcribed within one hour.
- Actively attends and encourages participation of patients in milieu activities.
- Identify, develop and implement client’s education programs.
- Orient the clients to the Clinic system and available services.
- Act as an advocate in directing clients to available Clinic and community resources.
- Negotiate appropriate treatment plans with clients.
- Educate clients regarding condition, treatment, and possible outcomes.
- Provide health care services for identified prevention and health promotion needs.
- Collaborate with other members of the staff, faculty and with volunteers.
- Evaluate and modify treatment as indicated by client outcomes.
Administrative:
- Maintain accurate medical records using EMR and proper forms in a timely manner.
- Train clinical staff and volunteers on data entry into the electronic health records system and provide oversight that original data is entered correctly.
- Consistently use supplies and equipment in a cost-effective manner.
- Recommend needed supplies and equipment for delivery of care.
Quality Assurance:
- Participate in quality improvement activities as directed by Director of Nursing and/or Chief Medical Director in coordination with the Executive Director.
- Communicate effectively with clients and their families in a respectful manner.
- Maintain an acceptable caseload to meet productivity needs.
- Utilize updated written protocols as mandated by the Arizona Board of Nursing Statutes and Regulations.
- Ensure that care provided is consistent with community standards of care within the limitations of the program.
- Follow established safety precautions in performance of duties to ensure a safe environment.
- Support policies and philosophy in delivering health care as set forth by Crossroads Board of Directors.
Qualifications:
- Valid, state-issued Nurse License in good standing.
- New graduates are welcome
- Clinical experience in a substance abuse, co-occurring and residential treatment patients’/clients setting preferred.
- Willingness to adhere to all in-house and legal regulations.
- Familiarity with EHR and EMR records systems.
- Excellent leadership, communication, problem solving and organizational skills.
- Current CPR/First Aid training.
- Ability to work independently and as a member of a team.
- Current and Valid State of Arizona Driver’s License Required.
