Hospice Clinical Supervisor / Care Coordinator

- Field supervision of hospice staff
- Assign patients and manage equitable territory assignments
- Coordinate care for new hospice referrals assuring equipment is ready in the home and staff is coordinated to meet the patient at their home upon hospital discharge; meet with hospitalized patients and their families to discuss care coordination and provide emotional support
- Conduct IDT and finalize IDT documentation
- Orientation of new staff to hospice
- Ongoing staff education in patient care, documentation, Medicare COPs, and levels of care
- Conference with potential hospice patients and their families regarding hospice services
- Participate in community functions to educate on hospice services
- Active participation in all committees
- Monitor and manage hospice expenses for appropriateness (DME & medications)
- Hospice admissions as needed
- Management of F2F schedules
- Other duties as assigned
- 2 years experience in nursing supervision required
- 3-5 years experience as a Hospice or home care case manager required, hospice preferred
