Reporting to the Nurse Manager/Supervisor, this role will coordinate with NPS providers and practice staff to provide ongoing support and communication to Chronic Kidney Disease (CKD) and/or End Stage Renal Disease (ESRD) patients as part of NPS's Value Based Care (VBC) team. The Coordinator acts as a single point of contact to coordinate resources for NPS VBC programs and NPS patients.

Must live in Pittsburgh area and be able to go into the office

Essential Duties and Responsibilities:

  • Perform care coordination activities, such as performing outbound calls and responding to inbound calls from patients, providers, and other teammates in regards to patient care. Completes non-clinical pathways & screenings. Escalates to AP or Provider as needed.
  • .
  • Assists with the referral process by pulling EHR data reports to review status, coordinating provider appointments and monitoring referral reports
  • .
  • Helps identify and manage program components onsite at practice locations. Works with front desk, medical assistants, physicians and other practice staff to coordinate survey administration and manage other activities as program requires.

  • Acts as subject matter advisor to teammates on navigating required program components.

Qualifications for the Care Coordinator:

  • Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook
  • .
  • At least 2 years of experience in a professional office environment
  • .
  • High School Diploma
  • .
  • Patient Care Technician or Administrative Assistant experience in dialysis preferred
  • .
  • Medical Experience highly preferred
  • .
