$22.00 per hour, Temp to Hire

Mon-Thurs, 8:00am-5:00pm (32 hr. work week- Fridays off!)

Grandview, Ohio (onsite with opp. to go hybrid)

What the position offers:
  • Position is onsite, but once proven, there may be some flexibility with a hybrid schedule

What you’ll be doing:
  • Using the Community and Pathways HUB model, coordinate the procurement of medical and social services for clients receiving care through this company
  • The Care Coordinator will also be a liaison among various medical, education, social service, and community agencies to optimize the coordination and provision of quality health care services
For patients who are cared through these programs, the care coordinator will do the following:

  • Coordinates linkage to medical home (primary care doctor) for patients receiving medical services through our programs. Schedules the initial appointment and conducts follow up calls to ensure proper connection to the medical home
  • Coordinates ancillary benefits for clients including prescription assistance, transportation, food benefits, public insurance, and interpreter assistance when necessary
  • Coordinates communication of services with clients and health care providers via telephone, mail, voice mail, email, or in person in a timely manner
  • Resolves issues and represents patient needs (e.g. financial assistance through hospital systems/medical bills, enrollment into public health insurance, etc.) to providers, service agencies, and assistance programs to ensure total wellbeing of clients
  • Develop and maintain relationships across the health care safety-net and with vendors of ancillary services that are essential to the coordination of health care services for patients
  • Comply with HUB guidelines to submit the completion of pathways, checklists, and tools in a timely manner
  • Attend all monthly HUB meetings
  • As a part of the care coordination team, maintains all forms and manuals related to our programs and is responsible for the timely distribution of updated information to our partners and website
  • Make recommendations for changes and improvements to our services
  • Maintains detailed records and prepares communications as needed
  • Interacts professionally on a regular basis with staff, vendors, and the general public
  • Provides leadership within the job and affiliated organizations as a whole
  • Assumes additional responsibilities as needed or directed
Who we’re looking for:
  • Associate degree in healthcare or social services related field preferred or equivalent community related experience
  • Minimum experience of one year in a community related or customer service field
  • Intermediate skill level Microsoft Office applications and use of intranet and internet
  • Must be confidential and knowledgeable in basic health care resources
  • Ability to read and write business correspondence for a diverse audience
  • Ability to organize work, engage in a variety of tasks simultaneously and consistently meet deadlines
  • High degree of initiative and independent judgment
  • Ability to express self effectively and concisely, both verbally and in writing
  • Ability to demonstrate a high attention to detail and good follow-up skills
  • Must be self-motivated, proactive, and solution focused
  • Ability to tactfully and effectively deal with public and staff in a personable and professional manner
  • Maintains a high level of energy and a consistent positive attitude
  • Ability to speak and write in Spanish or Somali languages is a plus
  • Interest in working with low-income and diverse populations
PREFERRED SKILLS; NOT REQUIRED!

  • Health care or social service experience in a nonprofit setting
  • College Degree in health care or social service area
  • Community Health Worker Certification
