POSITION SUMMARY
The LPN Community Care Coordinator is responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs while meeting with members in their homes. As a Community Care Coordinator (CCC), your goal is to help improve the quality of life and health literacy. The CCC helps to coordinate, monitor, and ensure information is shared in an appropriate and timely primary, across the continuum of care. The CCC will promote the effective utilization of tools and monitor healthcare resources.
DUTIES & RESPONSIBILITIES
- Provide care coordination field-based services to the assigned client population
- Provide care coordination to alleviate identified needs related to behavioral, physical, and social issues.
- Focus on addressing issues that will decrease medical costs and increase the quality of life.
- Develop positive relationships with clients, caregivers, providers, and other team members.
- Interact with peers in a collegial manner, supporting efforts to improve outcomes.
- Managing physical health problems through the provision of education
- Connecting with other community services
- Identification of dietary concerns
- Home safety evaluations
- Safe medicine use
- Education on specific illnesses and treatments
- Stress management
- Emotional support
- The monitoring process of clients through home visits
- Compliance with HIPAA regulations
- Maintain accurate records of care coordination in the EMR
SKILLS & QUALIFICATIONS
- One (1) year of experience as an LPN is preferred
- Experience in the care of the elderly in some geographic regions is preferred
- Experience in home health, physician’s office, or public health is preferred
- Computer skills required: (Microsoft Office Suite Software)
- Adapts to changes in policies, procedures, new techniques, & additional responsibilities
- Good driving record
- Knowledge of HIPPA regulations
- Bilingual (English/Spanish) is preferred
EDUCATION, LICENSES, & CERTIFICATIONS
- High School Diploma or equivalent required
- Bachelor's Degree preferred
- License Practical Nurse current and in good standing for Arizona
- Current CPR/BLS certification
- Annual TB testing
