GENERAL DESCRIPTION SUMMARY
The Admissions Registered Nurse (RN) in accordance with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Admission Nurse makes initial nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.
Essential Duties & Responsibilities - Within the philosophy, objectives and policies of Capital Caring, carries out the following essential functions:
Quality Provision of Services:
1. Initiates appropriate referrals to other disciplines and/or community.
2. Implements approved emergency medications appropriately and effectively.
3. Initiates preventive and rehabilitative nursing procedures to maximize quality of life.
4. Functions as a member of the regional and admissions team by demonstrating flexibility in work schedules and shared responsibility for delivery of care.
5. In collaboration with the Clinical Supervisor and Executive Director, conducts orientation, teaching, mentoring, and training or supervision of the orientation and training of Hospice employees and volunteers assigned to the clinical service.
6. Participates in quality improvement activities, QUAPI program and hospice sponsored in-service training. Serves on committees or teams as approved by the clinical manager.
7. Assumes responsibility for own professional growth and development in order to improve, maintain and develop new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc.
8. Evaluates own needs for support and uses identified system(s) to meet the need.
Organizational/Regulatory Compliance:
1. Explains hospice philosophy, goals, and services to patients and families to ensure their understanding of Capital Caring Health’s program.
2. With Team Social Worker, when present, secures admission to Capital Caring Health.
3. Chooses appropriate Hospice program to best meet patient/family needs.
4. Initiates the patient’s plan of care in accordance with nursing needs specified in the approved plan of treatment and in collaboration with patient/family, primary physician and other members of the interdisciplinary team. Identifies problems, goals, and interventions.
5. Uses the nursing process to meet the needs of the patient/family. Develops a plan of care with the patient/family and the other members of the interdisciplinary team.
6. Documents care provided into the medical record, including clinical information, nursing services delivered, communications and physicians orders in a timely manner using appropriate forms/formats according to Capital Caring Health policies/procedures.
7. Teaches and supervises the teaching of family/household members/persons who are responsible for assisting the patient with his/her nursing or personal care needs to empower them to meet the patient’s needs.
8. Prepares and coordinates clinical notes on nursing services delivered in order to document care into the medical record.
9. Delivers, or supervises the delivery of, skilled nursing services as outlined in the plan of care with focus on palliative, preventive, and rehabilitative nursing care and according to Hospice policies/procedures.
People/Communications:
1. Maintains communication with the patient/family, physician, RN Case Manager, patient accounts, team assistant, on-call staff, financial case managers, vendors/agencies and other members of the interdisciplinary team, as needed, to ensure the procurement of services and supplies necessary to implement the plan of care.
2. Collaborates with community agencies to coordinate plan of care.

1. Achieves expected productivity standard (based on patient visits, coordination, and discharge planning) as defined by the organization and in alignment with best practices/the Capital Caring model.
