Description
To provide nursing care to the residents as prescribed by the physician and in accordance with established facility protocol, local, state and federal regulations.
Duties and Responsibilities
1. Provide treatments to residents in accordance with physicians’ orders and established facility protocol and document accordingly. Including, but not limited to, treatment of decubitus, skin tears, and wounds.
2. Measure the size and stage of all decubitus on a weekly basis and document accordingly.
3. Prepare a weekly skin report for administration.
4. Audit skin assessments as needed
5. Complete or assist with the completion of weekly skin audits on residents who are at risk
6. Notify the Infection Control/Quality Assurance Nurse of any new infections, skin tears, or pressure ulcers
7. Contact the physician for initial orders and order changes as indicated.
8. Notify the responsible party of pressure ulcers, skin tears, or wound infections and document notification in the medical record in accordance with established facility protocol.
9. Be observant for changes in a resident’s condition and notify the supervisor and/or staff nurse accordingly.
10. Complete weekly foot care and assessment of diabetic residents and document in the treatment record accordingly.
11. In-service staff on treatment/skin care issues as needed.
12. Check on supplies daily and inform the individual responsible for ordering supplies of any supplies needed for resident care.
13. Order medications from the pharmacy as indicated.
14. Provide routine nursing documentation.
15. Check the Twenty Four Hour Report daily in order to be aware of
changes in the residents’ condition and/or plan of treatment.
Requirements
Licensure Requirements
Must be an L.P.N. currently licensed and in good standing with the Board of Nursing.
