Job Details
Responsibilities:
- Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified patient population.
- Engages patients and their families or caregivers in understanding, setting, and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
- Analyze patients medical history, medication allergies, physical condition, and examination to determine best care
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists or to internal medicine as appropriate.
- Directs and coordinates the patient care activities of nursing and support staff as required.
- Respect and promote patient rights and privacy.
- Responding appropriately to emergency situations.
- Participate in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts.
- Participate in specified health promotion, education and/or prevention programs.
- Participates in population health activities as directed by the practice.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards

Salary: $130,000.00 - $230,000.00 per year
