What are the responsibilities and job description for the Program Coordinator - Aftercare position at DreamTree Project?
Position Status: Full-time nonexempt regular employee at 36 hours per week
Aftercare Coordinator:
- Provide service with integrity, respect, empathy, and confidentiality to
youth / families;
- Create a safe and structured environment to provide service that is
trauma-informed and encourages positive youth development.
Education & Experience Requirements:
- High school diploma/G.E.D and a combination of two years education/training with
the target population and relevant experience in a human services related field.
Primary Responsibilities: planning
- Establish a collaborative project development team that includes, at a minimum: two
young adults from the target population(s)
- Identify all equipment & supplies needed
- Explore potential program models
- Coordinate with outreach staff to publicize services available.
- Provide case management services and resource connection.
- Supervise youth advisory staff and case manager/success coach in providing services.
- Develop and maintain a program schedule of youth-centered drop in times and
activities.
- Create an aftercare plan with each participant
- Meet with participants 1-1 to support their needs and goals
- Maintain clear, complete, and timely file documentation for each young adult.
- Supervise youth in accordance with DreamTree Project policies and procedures.
- Maintain facility security (e.g., lock and close all doors and windows when leaving).
- Provide transportation using DTP vehicle when relevant for program activities.
- Communicate facility maintenance needs promptly.
- Maintain program budget
- Attend meetings as scheduled, including weekly staff meeting and training, reflective
supervision, 1-1 meetings with supervisor, and external meetings with funders.
- Participate in the on-call phone rotation schedule.
- Maintain a clean driving record based on DreamTree Project insurance requirements.
