Summary: Incumbent performs duties according to standard Division policies, procedures, goals and objectives, exercising discretion and independent judgment in effectively applying guidelines to individual cases and circumstances and in supervising assigned personnel. Incumbent makes a substantial contribution to the agency's mission by directing effective training programs according to agency policies and procedures. Incumbent works with minimum direct supervision, referring to supervisor any unusual or unprecedented situations, such as misconduct of assigned staff. Work is periodically reviewed for compliance with agency policy and effect on agency goals and objectives. Incumbent maintains frequent contact with other Easterseals Arc personnel, other agencies, government officials, the public, clients and their families/guardians for the purpose of exchanging information, coordinating, directing and supervising training operations to meet client needs.
Duties and Responsibilities:
1. Ensures services provided are person-centered.
- Supports individuals served by Manufacturing to identify their unique strengths and the manner in which they would like to be supported so that each person has a day filled with meaning.
- Participates in team meetings to develop the Individual Support Plan (ISP) including the formation of meaningful and person centered goals that supports the individual toward greater independence.
- Completes assessments, as needed, to determine progress made by individuals on their goals and identifies barriers to success with recommendations to address those barriers.
- Maintains records of each individual on assigned caseload, including diagnosis and the limitations or barriers that the diagnosis creates for employment opportunities.
- Assures that direct support professionals implement goals and program objectives in a person-centered manner.
- Mentors all staff members to implement internal and external meaningful experiences for participants.
- Reviews monthly progress reports to ensure that progress is accurately documented and makes revisions as needed to ensure success of goals and desired outcomes for each assigned person on his/her caseload.
- Ensures detailed case notes are maintained on each individual and that billing/documentation records are accurate.
- Reviews case notes of direct support professionals to ensure that interactions are meaningful and relationships are positive.
- Provides crisis intervention/supportive counseling as needed with an empathetic and honest approach.
- Monitors each program area for issues and works with participants, direct support staff, and management to propose solutions and resolve issues.
- Provides direct support as needed to ensure that daily operations are effective, efficient, and illicit desired outcomes.
2. Develops a workforce that fosters employees who are engaged, honest, good communicators, willing to try new things, and positive role models for the people we support.
- Supervises assigned personnel, including interviewing, hiring, orienting, training, disciplining, completing performance reviews, and recognizing each staff member as appropriate.
- Monitors staff schedules, attendance, and leave time. Performs duties of absent personnel as needed.
- Monitors the development of each assigned staff member and makes recommendations for opportunities for growth.
- Provides staff training on each participant’s current ISP, BSP and risk plans. Ensures staff members are properly trained prior to working with participants and that the training is documented according to department procedures.
- Creates a climate in which people can do their best and makes each individual feel their work is important.
- Communicates information people need to do their jobs and clearly assigns responsibility for tasks.
- Creates strong morale and spirit in the team by sharing successes and soliciting the team’s opinions and input.
- Conducts team meetings on at least a quarterly basis.
3 Supports people to be safe and have optimal levels of health and well-being.
- Identifies training related to the health and safety of participants and ensures staff members are trained as needed, on at least an annual basis.
- Communicates with families and other providers to ensure the medical and emotional needs for each participant is met.
- Monitors the medication administration records for each participant to ensure that documentation is accurate.
- Completes incident reports, including BDDS incident reports, as required.
4. Completes administrative duties and positively represents Easterseals Arc.
- Trains and assists Direct Support Professionals (DSPs) in the operational compliance of regulatory bodies including CARF, Medicaid Waiver, and ICF/MR as well as PDG standard operating procedures and Easterseals Arc policies.
- Tracks data and maintains records for completing administrative reports as requested.
- Maintains confidentiality of records/information according to HIPAA, State, Federal Laws and guidelines.
- Maximizes and responsibly utilizes agency and community resources.
- Serves as a liaison to participants, families and community resources as well as working on special committees.
- Upholds agency values and maintains a professional demeanor inside and outside of the organization.
- Completes other duties as requested or assigned.
Qualifications:
- At least 18 years of age
- High School Diploma or GED required.
- Experience working with individuals with developmental disabilities; prefer 2 years.
- 1 year supervisory experience preferred. Experience in a workshop setting preferred.
- Ability to pass criminal/registry background checks.
- A valid driver’s license, proof of insurance and safe driving history is required. A working, insured vehicle is required.
- Ability to communicate verbally and in writing. Position requires computer skills for completion of reports and billing.
- Ability and willingness to support individuals with challenging behaviors and/or medical concerns.
- Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training and other trainings as deemed necessary by the agency.
Working Conditions: QIDP’s work in a changing environment that adjusts to the needs of the individuals in program services. The environment may be fast-paced with interruptions, distractions and deadlines. QIDP’s need to be flexible and be able to adjust to changing needs, activities and locations. QIDP must be able to tolerate tobacco smoke and/or pet dander based on the individual being supported. QIDP’s must have an insured, safe vehicle in good working order to utilize for incidental transportation required for staff and program participants to and from community activities.
PHYSICAL REQUIREMENT- Depending on the needs of the individuals, the work can be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 75 lbs, walking or standing for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions. Incumbent may work with violent/physically aggressive participants and responds to emergency situations involving potential physical harm to self and others.
