Essential Job Functions:
- Train, supervise, evaluate and monitor staff and Intermediate Care Facilities.
- Establish and maintain relationships with consultants who provide therapeutic services to residents. Contact those consultants for issues that arise.
- Ensure compliance with state certification/licensing standards, agency policies, other governing agencies guidelines and contractual obligations.
- Ensure compliance of documentation requirements as applicable (i.e. data, daily and monthly financial reports, monthly fire drills and other pertinent documentation).
- Attend client-centered conferences, team meetings, agency and staff meetings and other meetings as requested.
- Schedule and facilitate staff meetings that include a robust training component.
- Develop and monitor individual treatment plans to include scheduling and coordinating of appointments as necessary.
- Provide emergency on-call services.
- Ensure participation in integrated community activities on a regular basis for all consumers.
- Provide direct care services in assisting residents in meeting all of their personal needs.
- Provide services to individuals with varied behavior issues, which may include physical aggression, verbal assault, inappropriate sexual comments, gestures and/or acts.
- Assist individuals with toileting and personal care needs when necessary.
- Maintain all required data relative to program goals and objectives, resident activities, incidents, etc.
- Accept, undertake, and follow-through on administrative tasks as assigned by the residence manager, QIDP, or administrator.
- Provide direct, in the moment, coaching and training to direct support professionals on best practices and trainings to implement the systems of care on behalf of served individuals.
- Provide ongoing, daily supervision, attention, care and support for residents in OPWDD certified homes.
- Maintain ongoing, cooperative communication with family members, service providers, community partners, peers, supervisors, clinical professionals and administration.
- Provide positive role modeling for staff and residents.
- Maintain AMAP certification.
- Ensure appropriate documentation is received, followed, documented, and passed along and follow-up is conducted post nutritional, medical, therapeutic or dental treatment.
- Follow through with supervisory requests and seek supervision when unclear about expectations.
- Participate in available training opportunities geared toward expanding knowledge of treatment methods.
EDUCATION AND EXERIENCE:
- Bachelor's degree in Human Services or related field and/or
- Two years experience with people with developmental disabilities
- Administrative and supervisory experience preferred
- Valid driver's license and reliable transportation
