Grand Village seeks a MDS Specialist to join its team.  Grand Village consists of 119 nursing home beds and is located in Grand Rapids, Minnesota. Responsibilities

The MDS Specialist is responsible for assuring timely MDS (Minimum Data Set) completion for assigned patients/units to meet deadlines, with a focus on accuracy, case mix maximization, and regulatory compliance. In addition, this position may be responsible for development and implementation of staff education programs for the facility.

 

Essential job responsibilities:

 

  • Coordinates patient admissions, including data collection regarding patient condition at time of admission.
  • Coordinates initial care planning process, including documentation in patient chart and other designated communication areas.
  • Observes direct care staff interactions with residents, and provides education, guidance and training to improve clinical care and accuracy of MDS coding. 
  • Completes Resident Assessment Protocol (RAP) documentation based on regulatory requirements, MDS information obtained, and in accordance with professional standards of practice.
  • Coordinates with the IDT to help assure care plans are comprehensive and completed on time.
  • Other duties as assigned.
  • Qualifications

    MDS Specialist

    Minimum Required Qualifications:

    • Degree from an accredited school of nursing
    • Current LPN or RN licensure in the state in which they are working.
    • Up to 3 years related experience.
    • Ability to communicate effectively in both verbal and written formats.
    • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations.

    Preferred Qualifications:

    • Prior senior care experience
    • Experience with medical coding and coverage requirements for various payer types
