What are the responsibilities and job description for the SERVICE COORDINATOR - Early Start position at ELARC?
Under direction of the Early Start Supervisor, the SC will use a family-centered approach to perform the initial
psychosocial assessment of individuals suspected of having developmental delay/disabilities; obtains or collects
information or data for use by interdisciplinary team in determining each individual's eligibility for services (including
presenting cases to the multidisciplinary team for Early Start eligibility); analyzes data and identifies barriers to the
applicant’s developmental progress; counsels applicants concerning the assessment process and encourages them and
their families to fully participate in coordinating and utilizing the resources available; acts as an advocate for the
applicant and performs related work as required. Will carry a caseload of individuals being evaluated for eligibility
of early start services and ongoing case management; initiates referrals for specialty evaluations; assesses availability
and effectiveness of recommended services; monitors to ensure completion of assessment process within timeliness;
identifies community service development needs; prepares written and verbal reports; provides outreach to
agencies/professionals regarding the assessment and eligibility determination processes. Responsible for developing,
implementing and evaluating the Individual Family Service Plan (IFSP) process in partnership with parents/family
members. Responsible for coordinating multi-agency teams for assessment and IFSP development. Participates in the
eligibility process for Lanterman services. Monitors service delivery to infants and preschoolers to ensure
coordination between service providers and compliance with the IFSP. Utilizes a family-centered approach to
facilitate family-driven services. Provides support and assistance to families who may be having difficulty with
diagnosis, assessment, or in accessing services for their child. May serve on internal and/or community committees as
assigned. Other duties as assigned by unit supervisor.
QUALIFICATIONS
Bachelor’s Degree in social work and at least one year of direct experience working with at risk and/or
developmentally disabled infants, toddlers, preschoolers and their families. Master's Degree desirable. Familiarity
with early child development and infant/family issues required. Knowledge of the early intervention system;
self-motivated and interested in professional development; ability to adapt to a changing service system; excellent
verbal and written skills. Computer Proficient. Must be bilingual in English/Mandarin/Cantonese,
English/Vietnamese, English/Spanish or English/Korean. Must have an automobile, a valid California Driver
License and present proof of auto insurance.
