Nurse (Community)– Hybrid Schedule – $1000 Hiring Bonus!
Seeking a full time/part time (37.5 hours/week) nurse. The CAE RN works within the Nursing Department. The role of the CAE RN is to make onsite visits to area Nursing Facilities to assess consumers for clinical eligibility assessments for Mass health payment and potential discharge planning.
QUALIFICATIONS:
1. A valid Massachusetts license to practice as a Registered Nurse and the recognition/understanding of the duties, responsibilities, and limitations under the license.
2. A Bachelor of Science degree in Nursing or Associates of Science degree in Nursing and at least one (1) year of clinical experience and/or nursing in a community or long term care setting or a Diploma RN and two (2) additional years of community-based nursing experience and nursing facility experience required.
3. Knowledge of nursing facility discharge planning preferred.
4. Knowledge of community options for long term services and resources preferred.
5. Strong organizational skills with a focus on attention to detail, ability to multi-task and prioritize as well as analytical and problem-solving skills and the ability to work as part of a team.
6. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.
7. Computer proficiency, including experience with Microsoft products (Excel, Office, Word, SharePoint) required.
· A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $1,000.
